We would like to inform you about our data processing activities and your personal data which we (Dentist Ayten Kocaoglu as “data supervisor”) process under Law on Protection of Personal Data no 6698 (“Law”).

This text includes the clarification text regarding personal data processing activities on www.aytenkocaogly.com website (“Website”), where our establishment provides information regarding dental treatments and services (“Services”) to patient prospects.

What personal data do we process and what do we do with it?

Contact and Online Appointment Activities

Your certain personal data may be processed if you contact us through a number of channels (calling our business phone or request to be called by us, e-mailing us, filling out an online form, having an online chat with us through a communication pop-up window on our website, contacting us through our social media accounts or online instant messaging applications) to utilize our Services.

When you contact us to request information on medical services we provide, we will record and process the following data for following reasons: whether the service you requested are among the Services provided by our Establishment, carrying out the communication and information process regarding your request, if your reason to contact is suggestions or complaints concluding your request and providing you feedback, making an appointment if you request one, arranging your visit period suitable for the Service you requested, your ID (name & surname) and contact (phone number, e-mail address) in case we need to contact you for a condition regarding appointment (change, delay, cancellation, etc.) customer interactions (communication subject, requested treatment type, your notes for us, appointment date and time).

If you choose to leave a comment on comment sections of our website, your comment may be published for advertisement and inducement purposes, without revealing your name and surname except your initials  (i.e. as in A***** B****). Submitted ID (your name and surname), contact (phone number, e-mail address), and customer interaction (your comment) data will be processed to contact you in need, and to respond to your complaints and suggestions, if any.

If you would ask a question on our website on a subject relevant to our services, you question may be published on our website as part of our advertisement and providing information activities to enable website visitors to see your question and its answer, without revealing your name and surname except your initials  (i.e. as in A***** B****). Your personal information such as your ID (your name and surname), contact (phone number, e-mail address), and customer interaction (your question and any answer provided by a doctor) data submitted with your question will be processed to contact you in need, and to respond to your complaints and suggestions, if any, and enable website visitors to see your question and its answer. If you wish your question to be answered by a doctor without getting published, please contact us via e-mail, or make an appointment with our office.

Legal proceedings

In order to be able to prove the information and services provided to authorities, including courts, execution office, Ministry of Health, treasury and other agencies and institutions, and carry out and practice all relevant rights and liabilities such as suing, replying, making demands and requests, taking objection, etc. your ID (your name and surname), contact (phone number, e-mail address), and customer interaction (date of interaction, provided information, means, contact channel, scope, and related records) data will be processed.

Public agencies and institutions: we will be sharing your personal data with relevant agencies, institutions, and authorized personnel, in order to fulfil our obligations to provide information, documents, and similar to relevant authorities, and exercise our legal rights to sue or reply accordingly.

1. What method do we use to collect your personal data and what are the legal grounds for it?

Your data will be collected by both automatic methods, such as contacting us through one of our communication channels (filling out an online information form on our website, making a comment or asking a question, starting a chat in a pop-up window) and/or registering your information on our patient monitoring system, entering your appointment information on our appointment registry, or registering your consent to mandatory clarification, or non-automated methods, such as physical communication from government agencies and institutions (notification, notice, etc.), or written registry on mandatory clarification.

Your data will be processed on the following grounds:

• On legal reason that “obligation to process data for legitimate interest of data supervisor, provided fundamental rights and freedoms of relevant person is not harmed in any way”, for Communication and Online Appointment processes, patient comments, and Q & A activities.

You can deliver your application in writing to the address given below, or by sending an e-mail from your e-mail account registered on our system. (We would like to remind you that said application must follow the principles outlined in Communique on Rules and Principles of Making an Application to Data Supervisor.)
